All applicants included in the list are advised to prepare both the original and e-copies (PDF format) of the following documents on or before 29 July 2025:

b. PSA Certificate of No Marriage (CENOMAR)

c. National Police Clearance

d. NBI Clearance

e. College Diploma

f. Transcript of Records (TOR)/ SF10 or Form 137-A

g. Professional License/ Certificate of Eligibility with General Weighted Average (GWA)

h. Certificate of General Point Average (GPA) issued by the School Registrar

i. NCIP Certificate (if applicable)

j. BIR Taxpayer Identification Number (TIN) ID

k. PhilHealth ID

l. PAG-IBIG ID

m. Endorsement Letter from Unit Commander (For Enlisted Personnel Only)

n. Latest Performance Evaluation Report (For Enlisted Personnel Only)
